Improve Tramp's user and host name completion

* lisp/net/tramp.el (tramp-completion-handle-file-exists-p):
Improve user name completion.
(tramp-skeleton-file-exists-p): New defmacro, which also handles
host name completion.
(tramp-handle-file-exists-p):
